Software development for the AT&T 5ESS(R) Switch employs design reviews, code inspections, and several levels of testing to verify and improve the quality of the developing software. This paper defines a model of the fault removal process developed for use in 5ESS(R) Switch software development, and outlines a set of in- process fault density metrics. Fault densities, based on the number of errors found at each of the verification stages, are used as metrics to track, and help manage, the quality of the developing software. Such a model could be useful on any large software development project.
